Overview
- A stay in hospital can have financial consequences
- Sometimes you need help from professionals which you can’t get on the NHS
- A root canal filling is even more painful when you get the bill and a broken pair of glasses can knock your finances off course
The health and wellness plan is designed to offer you financial assistance for everyday and emergency medical costs.
You can choose from 4 different cover levels and get cover for you and your partner if you’re both:
- Aged between 18 and 69 years old inclusive
- A resident in the UK and living at the same address
You can add your children for free (up to the age of 21 if they still live with you or are in full time education)

Premiums
The premiums for each level of cover are:

Great! Please make sure you have read the Important Information Document and Policy terms and conditions before applying.
Once you have sent in your application you will receive your policy documents within 5 working days. Your certificate of insurance will show your policy start date. This is when you are covered from and is the date claims can be made from. This date can be anything up to 30 days in the future depending on your payroll department’s or the Direct Debit processing times.
You can return completed application forms by email or post. If returning by email, please attach as a PDF document and send to customerservices@myhealthxtras.co.uk. Alternatively, to return by post please send to: Customer Services, My Health Xtras, 2 Charlesworth Court, Knights Way, Battlefield Enterprise Park, Shrewsbury SY1 3AB.
